Autoantibodies directed against deamidated gliadin peptides or against deamidated gliadin protein epitopes are referred to as anti-DGP. The anti-DGP antibody ELISA assay is a newish test, replacing the older test for autoantibodies to native gliadin, which did not provide sufficient specificity for a conclusive diagnosis of celiac disease.
Test systems for quantitative detection of antibodies against DGP may support the formation of a diagnosis when celiac disease is suspected. Testing for anti-DGP antibodies may also be positive in some people with celiac disease who are tissue-transglutaminase antibodies (anti-tTG) negative, including children less than 3 years old.
In sera of celiac disease patients both anti-DGP antibodies of the IgA class and anti-DGP antibodies of the IgG class can be detected. As a significant number of CD patients are IgA deficient, sensitive screening for risk groups includes testing for both IgA and IgG antibodies. By IgA antibodies quantification disease activity can be estimated, and the maintenance of a gluten-free diet can be monitored.
