Double-stranded DNA antibodies (anti-dsDNA)

The anti-dsDNA test detects autoantibodies directed against DNA, the genetic material inside the cell nucleus. Anti-dsDNA antibodies belong to the group of antinuclear antibodies (ANA), and are primarily associated with systemic lupus erythematosus (SLE). A positive anti-dsDNA test is highly specific for SLE but only about 50-60% of SLE patients produce anti-dsDNA antibodies.

Lupus nephritis, an inflammation of the kidneys, which can lead to protein in the urine, high blood pressure, and kidney failure, is a serious complication of SLE. It occurs when immune complexes consisting of the autoantibodies bound to their respective antigens become deposited in the kidneys. In patients with lupus nephritis, a high titer of anti-dsDNA is generally associated with ongoing inflammation and damage to the kidneys.

The anti-dsDNA test may be used to monitor disease activity in a patient diagnosed with SLE. Increased anti-dsDNA levels may be seen prior to and during flare-ups.
